McKnights Home Care: Organizations push for new fee-for service, community-based palliative care model

In a recent interview with McKnight’s Home Care, our Founder & CEO Tom Koutsoumpas spoke to a reality many providers see every day: patients with advanced illness who are not yet eligible for hospice, but clearly need support to remain at home.

Today, there is no comprehensive, sustainable fee-for-service pathway in traditional Medicare to meet that need.

Together with the The Coalition to Transform Advanced Care (C-TAC), NPHI is working to change that. We are advancing the development of a federal, community-based palliative care model that expands access to care earlier in the illness trajectory, supports patients and families in the home, and helps reduce unnecessary hospital utilization.

This is not a new idea. It is a long-recognized gap in the system. What is different now is the urgency and the alignment to address it.

We look forward to continuing this work with partners across the healthcare system and with leaders at Centers for Medicare & Medicaid Services to help bring a sustainable model to life.
