Job Description: – Provide recommendations about symptom management and facilitate goals of care conversations for seriously ill patients in medical, surgical, and ICU settings
– Participate in daily team rounds
– Communicate with care teams to facilitate high-quality, compassionate care
– Evaluate patients for hospice eligibility, conduct inpatient hospice (GIP) admissions, and facilitate discharges home with hospice
– Thoroughly document visits with adherence to billing practices
– Participate as a member of the hospital’s interdisciplinary team, including representing palliative/hospice care in committees as applicable
– Conduct formal and informal education about palliative and end-of-life care
Salary (optional): $110,000-$130,000
