NPHI Expands its National Partnership with Four New Exceptional Nonprofit Members

NPHI welcomes four leading nonprofit care providers to the community, expanding the partnership’s collective impact on advanced illness and hospice care nationwide.

The National Partnership for Healthcare and Hospice Innovation (NPHI), together with its growing network of nonprofit, community-based providers, proudly welcomes Community Hospice, Everent Health and its affiliate Bluegrass Care Navigators, Treasure Health – Treasure Coast Hospice, and Visiting Nurse and Hospice for Vermont and New Hampshire (VNH) as its newest members.

As part of NPHI’s collaborative movement to advance person-centered care across the continuum of serious illness, these organizations will work alongside more than 120 nonprofit members to improve healthcare access, quality, and healthcare innovation to provide the highest quality care to millions of patients and families across the nation.


Community Hospice

Serving seven counties in Kentucky and two in Ohio, Community Hospice provides skilled, compassionate care to patients of all ages living with terminal illnesses. The organization offers home-based hospice, inpatient care, palliative care, comprehensive bereavement support for adults and children, plus many additional services.

As an independent, nonprofit provider licensed in both states and certified by Medicare, Medicaid, and The Joint Commission, Community Hospice delivers care that meets the highest standards of quality and accessibility. The organization proudly holds The Joint Commission’s Gold Seal of Approval, a symbol of excellence in healthcare.


Everent Health and Bluegrass Care Navigators

Everent Health is building a sustainable future for community-based care providers. With decades of experience in nonprofit hospice and serious-illness care, Everent’s team of industry experts helps organizations thrive through collective resources, modern technologies, and streamlined processes. Founded in 2025, Everent represents a strategic partnership between two of the nation’s largest independent providers of hospice and serious-illness care: Hosparus Health and Bluegrass Care Navigators (both now NPHI members).

For more than four decades, Bluegrass Care Navigators has served as a trusted leader in serious illness care across 32 counties in Kentucky, operating seven regional offices and two inpatient units. Their broad range of services, including Bright Path Dementia Care, Grief Care, Hospice, Integrative Medicine, PACE, Palliative Care, and Transitional Care, reflects their commitment to improving comfort, quality of life, and peace of mind for patients and families at every stage of an illness.

Rooted in community since 1978, Bluegrass continues to evolve through innovative care models that help individuals navigate complex health challenges with excellence and compassion.


Treasure Health – Treasure Coast Hospice

Founded in 1982, Treasure Coast Hospice serves patients and families in Martin, St. Lucie, and Okeechobee counties, Florida. Guided by a vision that everyone deserves choice and dignity in how they live at every stage of life, the organization provides expert hospice and grief support services delivered with compassion and respect. Treasure Coast Hospice along with existing member St. Francis Reflections Lifestage Care are services of Treasure Health.

Through open communication and personalized care, Treasure Coast Hospice empowers patients and families to plan for and navigate serious illness with confidence and peace of mind.


Visiting Nurse and Hospice for Vermont and New Hampshire

Visiting Nurse and Hospice for Vermont and New Hampshire is home to one of the foremost teams of home health experts serving more than 140 towns across Eastern Vermont and Western New Hampshire. With over a century of local experience, VNH delivers exceptional home health, hospice, maternal child health, personal care, and support group services with integrity, compassion, and proven effectiveness.

As a nonprofit organization and proud member of Dartmouth Health, VNH is uniquely positioned to impact the growing demand for home-based care. Its deep community roots trace back to the local home nursing movement that began in Windsor, Vermont in 1907—making VNH one of the region’s oldest and trusted healthcare providers.

VNH’s mission is simple and powerful: to deliver the highest quality care where patients heal best—at home.
