The National Partnership for Healthcare and Hospice Innovation (NPHI) is proud to welcome two mission-driven nonprofit providers — St. Anthony’s Hospice and Transitions LifeCare — to its growing national membership of community-based organizations committed to reimagining care for people living with serious illness.
St. Anthony’s Hospice: Four Decades of Compassionate Care in Western Kentucky
St. Anthony’s Hospice, founded in 1982, has been a trusted community resource for individuals and families in Henderson, Union, and Webster counties for many years. Over the last four decades, St. Anthony’s has continued to evolve to meet community needs — expanding care delivered at home and in long-term care facilities, establishing the Lucy Smith King Care Center in 2009, and launching a Transitional Care program in 2016 that has since grown into a full palliative care program.
Today, St. Anthony’s offers hospice care, palliative care, and bereavement services, grounded in a belief that “the end of life is as precious and fragile as the beginning.” The organization continues to honor its founders’ vision: delivering exceptional, compassionate care to every individual and family they serve.

Transitions LifeCare: A Legacy of Community-Based Support Since 1979
Founded in 1979, Transitions LifeCare has grown into one of North Carolina’s most trusted nonprofit providers of hospice care, palliative care, grief support, and caregiver services. Guided by a clear mission to care for individuals and families with compassion, support, and expertise, Transitions LifeCare helps patients navigate serious illness and end-of-life care with dignity and comfort.
The organization serves communities across Chatham, Durham, Franklin, Granville, Harnett, Johnston, Nash, Orange, Wake, and Wilson counties. With a philosophy that everyone deserves a meaningful quality of life, Transitions LifeCare focuses on “the right care at the right time,” ensuring patients and families experience the support they need most.
