Advanced Cardiac Care Program™
A resource guide for patients and their caregivers/family members
Our Advanced Cardiac Care Program™ helps patients, caregivers, families, and providers understand and manage serious heart conditions. It offers simple, helpful information and tools to make care easier and safer—often from the comfort of home. The goal is to support people living with heart disease so they feel more confident and cared for every step of the way.
Helping People with Heart Disease Live with Comfort, Support, and Dignity
A collaboration between NPHI and the American Heart Association
Heart disease is still the number one cause of death in the United States. Many patients—especially those living with heart failure—don’t know about the care and comfort hospice and palliative care can offer. That’s something we’re working hard to change.
At NPHI, we’ve partnered with the American Heart Association to make sure heart patients and their families have access to the right care at the right time.
