Our session provided an exciting opportunity to listen and learn what many individuals across the membership have been working on with the goal of raising the collective standard of care across NPHI member programs.
NPHI’s Innovation Lab team provided an update and basic orientation to the initiatives currently underway, and the resource guides previously developed. Specifically, the presentation included a reminder of the purpose of the Innovation Lab, related to Quality, Standards, Access, and Innovation, and touched on the work they’ve done with respect to:
- advanced cardiac care
- community counseling
- veteran’s supports and services
- advanced lung disease.
Initiatives are developed and implemented with four pillars in mind:

- Internal SME Direction
- External SME Validation
Released Initiatives
- Advanced Cardiac Care (ACC) Program developed in collaboration with American Heart Association
- Community Counseling, NPHI iSMEs developed a resource guide of best practices that guides a program through the essential steps necessary to establish or enhance and operate a sustainable community counselling program.
- Veteran’s Services, a collaborative effort among NPHI programs to establish a set of best practices for all member programs to furnish core veteran’s supports and services.

The National Partnership for Healthcare & Hospice Innovation (NPHI) is a collaborative of 90+ not-for-profit, community-integrated hospice and palliative care providers dedicated to ensuring patients and their families have access to care that reflects their individual goals, values and preferences.
